Salam sejahtera. Apa khabar semua? Harap-harap korang dah baca cerita pasal Hospital Klang semalam. Dah boleh hadamkan semua maklumatnya? Jom kita sambung lagi penceritaannya. Kali ini kita analisis pula apa yang berlaku setelah bangunan -bangunan baharu serta wad-wad dibina di situ...JOM.
B. PEMBINAAN HOSPITAL DAERAH KLANG YANG KEKAL
Satu laporan tahunan daripada Jabatan Perubatan Hospital Klang, pada Februari 1886. Laporan itu membincangkan topik berikut: rumah mayat telah diubah suai, kakitangan terdiri daripada seorang jururawat, atendan, dan tukang masak, penginapan hospital masih diperlukan, 175 kes telah dirawat di Hospital dengan kadar kematian sebanyak 2.286%, dispensari menggunakan Hospital Kusta sementara, dan kerja-kerja penyambungan di Penjara telah selesai. Laporan itu menekankan keperluan untuk penambahbaikan kepada infrastruktur dan kakitangan hospital.
Secara khusus, laporan itu menyerlahkan isu berikut:
1. Keadaan bangunan utama hospital kurang baik, bumbung bocor dan struktur kayu reput.
2. Wad telah dinaiki air semasa hujan lebat, dan bumbung perlu dibaiki.
3. Bilik stor hospital telah diperbesarkan, tetapi masih ada keperluan untuk kemudahan penyimpanan yang lebih baik.
4. Tandas masih kurang memuaskan, tetapi tandas kekal sedang dirancang.
Justeru suatu tapak baharu yang lebih selamat dan kering dikenal pasti untuk kerja-kerja pembinaan hospital daerah yang baharu.
1. Pembangunan Infrastruktur dan Kelengkapan Asas
a. Hospital Besar Klang
Sepucuk surat dari Pejabat Kerja Raya di Kuala Lumpur (Ogos 1887) memberitahu kontrak pembinaan Hospital Klang dengan harga $8,000. Surat itu menyebut bahawa projek itu telah dibincangkan secara lisan, dan undian akan digabungkan untuk pembinaan hospital baru, kedai dispensari dan kuarters, serta dua dinding bata.
Surat bertarikh 18 Julai 1889, yang ditulis oleh John Chelch, seorang Pakar Bedah Residensi, kepada Residen British Kuala Lumpur mencadangkan pemindahan 100% peruntukan untuk "Keselesaan Perubatan" dari Kuala Selangor ke Hospital Daerah Klang, dengan alasan ketidakcekapan dalam penggunaan dana. Ia memberikan pandangan tentang aspek pentadbiran dan kewangan penjagaan kesihatan di Selangor semasa era penjajahan British, menonjolkan kepentingan peruntukan sumber dan penggunaan dana yang cekap dalam perkhidmatan kesihatan awam.
Pada 26 Mac 1891, Jurutera Daerah memberitahu penyiapan kerja di bawah Kontrak No. 20 untuk pembinaan longkang bata dan tangga di Hospital Daerah Klang. Laporan itu menyatakan bahawa kerja telah disiapkan dengan memuaskan dan Pegawai Daerah Rendah mengesahkan kerja telah siap. Laporan itu mengesyorkan membayar kontraktor bayaran akhir untuk kerja yang telah siap.
Pada tahun 1892, British mula membina Hospital Daerah Klang secara berperingkat, menandakan permulaan hospital formal di Selangor. di kaki bukit antara Jalan Kota (Fort Road) dan Jalan Istana (Sultan Street). Berhampiran dengan hospital ini, terdapat sebuah gereja Roman Katolik, serta sekolahnya (masih wujud sehingga kini). Pegawai Perubatan (District Surgeon to the Coast District) pertama yang bertanggungjawab ke atas hospital ini ialah Dr.Watson diikuti oleh Dr. Reid (16/1/1908 hingga 25/3/1908) dan seterusnya Dr. Gerrard dan Dr. Millard.
Surat daripada Pakar Bedah Residensi bertarikh 22 Oktober 1892 memajukan cadangan untuk menggunakan dua pemotong rumput untuk memotong rumput tinggi (lalang) di Hospital Klang. Pakar Bedah Residensi bersetuju bahawa ini adalah perlu dan mencadangkan bahawa gaji tukang kebun untuk dua bulan akan datang dibayar untuk menampung perbelanjaan.
Pakar Bedah juga mengesyorkan supaya Apoteker dibenarkan mengupah seorang peon (pembantu berpangkat rendah) untuk tahun 1890 dan membayar lebihan daripada elaun atendan. Dia juga mencadangkan mengupah seorang pelayan prom (pekhidmat) kerana jarak antara hospital dan pelbagai pejabat.
Satu surat daripada Jurutera Negeri kepada Setiausaha Kerajaan Selangor, bertarikh 22 November 1895. Surat tersebut membincangkan kualiti kerja-kerja pelebaran Dapur Hospital Klang. Jurutera Negeri mengesyorkan denda $50 untuk digunakan bagi membetulkan kecacatan dalam kerja. Lanjutan kontrak diberi untuk membolehkan kontraktor menambah masa untuk menyiapkan kerja.
Tender pembinaan tandas di Hospital Klang di Selangor menyenaraikan empat tender daripada syarikat atau individu yang berbeza dalam tahun 1900. Dokumen ini menyediakan maklumat penting tentang setiap petender, termasuk nama mereka, jumlah wang yang mereka sebutkan untuk projek itu, keselamatan yang mereka sanggup sediakan untuk memastikan projek itu siap, dan anggaran masa yang diperlukan untuk menyiapkan projek.
Mengikut statistik, pada tahun 1901, seramai 2789 pesakit luar dan 1822 pesakit dalam telah dirawat di hospital ini. Ketika itu, kes cirit-birit, penyakit pernafasan dan penyakit Kanak-kanak adalah kes utama yang dirawat di hospital ini.
Satu memorandum rasmi daripada Jurutera kepada Setiausaha Residen, bertarikh 20 Mac 1905 mengumumkan notis memanggil sebut harga bagi pembinaan wad pembedahan dengan 10 katil di Hospital Klang di Selangor. .
Surat daripada Jurutera Negeri Selangor kepada Setiausaha Residen, bertarikh 15 Februari 1907 memohon kebenaran untuk memulakan projek menyediakan bekalan air baru ke Hospital Daerah Klang. Projek itu melibatkan pemasangan paip 3 inci dari tangki berhampiran kuarters Pegawai Daerah ke bangunan hospital. Anggaran kos projek ialah $4000, dan inden untuk bahan sedang dihantar kepada Ejen Mahkota. Projek ini dianggarkan menelan belanja $4300, dan perbelanjaan akan dicaj kepada belanjawan tahunan.
Pada 10 Jun 1907, tender untuk kerja-kerja kecil membina tandas di Hospital Klang di Selangor menggariskan skop kerja, syarat dan syarat pembayaran.
Surat bertarikh 28 Jun 1907, dari Pejabat Jurutera Negeri di Kuala Lumpur kepada Setiausaha Residen di Selangor yang merujuk surat-menyurat S.R.2341/07, sebelum ini mengenai pembinaan wad baharu di Hospital Klang. Jurutera Negeri sedang mengemukakan kontrak kerja sekeping untuk penggalian yang akan digunakan untuk mengisi tapak untuk Dispensari baharu, dan batu laterit yang digali akan digunakan untuk tujuan penyelenggaraan jalan.
Dalam tahun 1911, kesesakan berlaku di Hospital Klang, yang kini direka untuk menampung 185 pesakit tetapi secara konsisten melebihi kapasiti itu dengan kemuncak 230 pesakit dalam satu hari. Terdapat kes di mana pesakit yang masih belum sihat didiscaj daripada hospital kerana kekurangan kemudahan yang sesuai.
Tender pembinaan wad pembedahan di Hospital Daerah Klang bertarikh 28 Mac 1912., disenaraikan sebagai $3400 00 (Tiga ribu empat ratus sahaja). Syarat-syarat tender termasuk keseluruhan pembinaan mestilah mengikut Spesifikasi Jabatan Kerja Raya, dan mesti siap dalam tempoh di bawah penalti $7 bagi setiap hari yang melebihi tempoh tersebut.
Pada tahun 1918, Hospital Klang mengalami peningkatan dalam bilangan pesakit yang memerlukan rawatan, terutamanya dalam bidang perbidanan. Untuk menangani situasi ini, pihak hospital menambah bilangan pegawai perubatan wanita dan jururawat untuk memikul tanggungjawab dalam bidang perbidanan. Langkah ini juga mencerminkan keperluan untuk meningkatkan mutu perkhidmatan kesihatan, khususnya dalam memberikan rawatan kepada wanita hamil dan melahirkan anak.
Jabatan Perubatan telah melancarkan program sukarela untuk menggalakkan penyertaan wanita tempatan dalam perkhidmatan perubatan, khususnya di daerah Klang. Program ini mendapat sambutan positif daripada penduduk setempat, menunjukkan bahawa masyarakat tempatan, termasuk wanita, berperanan aktif dalam meningkatkan perkhidmatan kesihatan di daerah mereka.
Kerajaan kolonial juga menyeru orang ramai untuk turut serta dan memberikan kerjasama dalam usaha memperbaiki perkhidmatan perbidanan. Seruan ini berjaya menarik sokongan daripada komuniti Cina di Klang, yang memberikan bantuan melalui penyediaan ubat-ubatan dan khidmat perbidanan yang dibawa dari negara asal mereka. Ini menunjukkan kerjasama antara pihak kerajaan kolonial dan komuniti setempat dalam usaha meningkatkan taraf kesihatan masyarakat di Selangor pada masa itu.
Keseluruhannya, Hospital Besar Klang di puncak bukit yang indah menghadap ke seluruh pekan. Reka bentuk ini adalah reka bentuk British pada awal abad ke-20 dan semua hospital Komanwel mempunyai hospital yang kelihatan seperti ini. Hospital itu terdiri daripada wad satu tingkat yang diperbuat daripada kayu dan berlantaikan simen serta beberapa bumbung berjubin dan sebahagian lagi bersalut zink. Bahagian tepi wad mempunyai banyak dinding berpusing yang boleh membuka keluar untuk membenarkan udara masuk. Sebuah klinik pergigian baharu dibina dalam tahun 1962 dan siap pada Mei 1962. Jumlah kos projek itu ialah $80,000, termasuk $71,000 untuk bangunan dan $9,000 untuk peralatan. Klinik ini akan menyediakan kemudahan yang lebih baik untuk pesakit dan akan dikendalikan oleh pasukan doktor gigi. Mulai 1 Januari 1976, hospital telah dinaik taraf kepada Hospital Besar Klang dan diketuai oleh Penguasa Perubatan.
18 September 1963 - KLANG. 10 Sept.-Hospital Daerah di sini mendapat rawatan muka dengan kos lebih daripada $30,000. Hospital kayu, dibina lebih 50 tahun lalu. telah berada dalam keadaan buruk selama beberapa tahun. Hospital 10 wad itu memberi perkhidmatan kepada kira-kira 350,000 penduduk Klang dan kawasan pantai
2. Pembangunan dan Pengurusan Sumber Manusia
Mulai tahun 1906, Jabatan Perubatan Selangor mempunyai beberapa jawatan penting, termasuk Pakar Bedah Negeri, yang bertanggungjawab untuk menyelia perkhidmatan perubatan. Terdapat juga lima Pakar Bedah Daerah, yang bertanggungjawab untuk kawasan tertentu, dan seorang Pakar Bedah Rumah, yang bertanggungjawab untuk operasi harian hospital.
Jabatan ini juga menggaji tiga Penolong Pakar Bedah, tujuh Apotekar, dan sejumlah 38 Dresser (19 Gred I dan 19 Gred II). Matron bertanggungjawab untuk mengawasi kakitangan kejururawatan, dan terdapat dua orang Jururawat yang bekerja di bawah pengawasannya.
Kakitangan pentadbiran termasuk seorang Ketua Kerani dan Pembantu Pejabat, yang bertanggungjawab menguruskan kertas kerja dan surat-menyurat jabatan. Terdapat juga dua orang Kerani (Gred III) yang membantu tugas-tugas pentadbiran. Selain itu, terdapat beberapa Kerani dan Steward yang bertanggungjawab untuk tugas tertentu seperti pendaftaran kelahiran dan kematian, serta menguruskan operasi harian hospital dan suaka tertentu. Sejumlah 55 kakitangan ini , dibahagikan kepada tiga kategori:
Kakitangan Perubatan: 34 kakitangan, termasuk Pakar Bedah Negeri, Pakar Bedah Daerah, Pakar Bedah Rumah, Penolong Pakar Bedah, Apotek, Dresser (Gred I dan II), Matron dan Jururawat.
Kakitangan Pentadbiran: 4 kakitangan, termasuk Ketua Kerani dan Pembantu Pejabat, Kerani (Gred III), dan Kerani Pendaftaran Kelahiran dan Kematian.
Kakitangan Khusus: 17 kakitangan, termasuk Penyimpan pakaian untuk suaka tertentu (Lunatic, Gaol, dan Leper), Steward untuk Hospital Besar, dan Kerani dan Penjaga Stor untuk pelbagai hospital dan daerah.

Dokumen bertarikh 15 Januari 1903 adalah Perintah Kerajaan (G.O.) bersejarah dari Negeri-negeri Melayu Bersekutu, . Ia menggariskan gaji dan kadar gaji untuk pelbagai jawatan dalam Jabatan Perubatan, Hospital, Klang, di Selangor. Jawatan yang disenaraikan termasuk Atendan & Memasak, Pengangkut air, Dhoby (pekerja dobi), dan Tukang Kebun. Dokumen itu menyatakan bahawa gaji akan diambil di bawah kuasa G.O. ini dan sebarang perubahan akan dilaporkan kepada Perbendaharaan dan Ketua Audit Negara FMS

Dalam tahun 1918, Hospital Daerah Klang mempunyai pegawai-pegawainya seperti berikut:

Klinik-klinik ini menyediakan rawatan percuma kepada semua pesakit, tanpa mengira kaum atau jantina. Laporan itu memuji kakitangan yang dilatih oleh pakar dan penjagaan yang diambil untuk merawat pesakit dengan pelbagai keadaan.
3. Tender-tender dan Pemerolehan Sumber
Secara keseluruhan, dokumen ini menyediakan gambaran terperinci tentang keperluan logistik hospital era kolonial, menunjukkan integrasi bahan dan amalan tempatan dengan yang dibawa oleh pentadbiran penjajah. Jadual tender daripada Jabatan Kerja Raya Selangor, bertarikh 1896, bagi pembekalan catuan dan peruntukan bagi tahun 1897. Jadual tersebut mengandungi maklumat seperti nama petender, cop mark, penjamin, saksi, dan wakil, serta jumlah tender, keselamatan yang ditawarkan, dan masa penyiapan.
Jadual tersebut merupakan kontrak antara Kerajaan Selangor dan pembekal bagi Hospital Daerah Klang bagi tahun 1897. Kontrak tersebut menggariskan terma dan syarat perjanjian termasuk spesifikasi pembekalan pelbagai jenis makanan dan barangan keperluan.

Pada 20 Mac 1912, notis telah dikeluarkan oleh Excel, Jurutera kepada Setiausaha Residen, Kuala Lumpur, yang ditujukan kepada Setiausaha Residen. Notis tersebut dimajukan untuk makluman penerima dan memohon sebut harga bagi pembinaan wad pembedahan dengan 10 katil di Hospital Klang.
Dokumen itu adalah panggilan rasmi untuk tender, meminta kontraktor mengemukakan cadangan mereka untuk projek itu. Selangor mempunyai 5 hospital di Kuala Lumpur dan hospital-hospital lain in Klang, Kajang, Kuala Kubu Baru and Serendah. Hospital-hospital di Kuala Lumpur ialah Hospital Besar, Hospital Bangsar, Hospital Melayu, Hospital Penjara dan Wad Polis di Tanglin. Pada tahun 1939, hospital-hospital di Selangor ini telah menerima 33,201 pesakit dan merawat 265,877 pesakit luar.
5. Isu dan Masalah
1. Adalah mustahil untuk kadar kelahiran dan kematian mana-mana populasi Imigresen dan emigrasi diperiksa dan angka tidak tersedia kecuali buruh ladang yang melalui kem kuarantin. Kematian direkodkan terhadap bandar dikira berdasarkan kediaman tiga bulan. Penduduk luar bandar datang ke bandar apabila sakit atau ke hospital yang terletak di bandar, oleh itu peraturan ini dikira dapat menunjukkan hasil yang lebih benar daripada kadar kematian kasar.
2. Kesilapan serius (Wabak Malaria) di Port Swettenham dan Klang sekitar 1895 hingga 1905 dapat dielakkan dan banyak wang dapat dijimatkan jika sepuluh tahun sebelum itu mereka menggajikan sekurang-kurangnya dua jurutera Sanitasi. Mereka mesti bekerja dalam hubungan rapat dengan pekerjai hospital, dan memastikan diri mereka dikemas kini dalam kerja klinikal.Satu jabatan perubatan dan penyelidikan perubatan mesti diwujudkan di jabatan itu. Kemudian entomologi mesti dikaji, dan kakitangan jurutera kebersihan tropika mesti dilatih. Hospital akan menjadi barometer kepada mereka.
3. Klinik Penyakit Kelamin yang pertama telah ditubuhkan pada tahun 1923, dan menjelang 1925, Klinik Kuala Lumpur mempunyai 2,328 kehadiran dan Dispensari Bandar mempunyai 29,335. Pusat tambahan sedang ditubuhkan di Ipoh, Seremban, Taiping, dan Klang, dengan rancangan untuk lebih banyak lagi pada masa hadapan. Sub-pusat juga boleh didapati di stesen yang lebih kecil. Pakar Penyakit Kelamin telah membangunkan graf yang menunjukkan kejadian penyakit kelamin di pelbagai lokasi, dan telah berjaya merawat kes granuloma inguinalae.
4. Menangani ketagihan candu memerlukan usaha yang rasional dan bersepadu untuk mengelakkannya daripada menjadi masalah yang lebih sukar untuk ditangani pada masa hadapan. Kepentingan publisiti dan keyakinan orang ramai dalam merawat ketagihan candu di Tanah Melayu. Walaupun sebuah bandar kecil seperti Klang boleh mendapat manfaat daripada tiga kali ganda penginapan semasa.
Justeru, Hospital Klang telah menawarkan perkhidmatan Wad Anti-Candu dalam tahun 1925. Pada 20hb Januari 1925, sebuah wad anti candu telah dibuka di Hospital Daerah, Klang, dengan 7 pesakit, tetapi kini terdapat 40 katil, telah diduduki sepenuhnya. Lebih ramai pesakit dijangka. Wad itu dijaga bersih. Kedua-dua Dr Gabriel dan dresser bekerja sehingga lewat untuk dua malam pertama kerana mendaftarkan nama pesakit. Idea wad anti-candu berkemungkinan menyediakan persekitaran yang selamat dan menyokong bagi individu yang bergelut dengan ketagihan candu untuk menerima rawatan dan pemulihan. Wad itu menawarkan rawatan perubatan, kaunseling dan perkhidmatan lain untuk membantu individu mengatasi ketagihan mereka dan berintegrasi semula ke dalam masyarakat. Perlu diingat bahawa konsep wad anti candu mungkin khusus untuk tempoh masa teks itu ditulis, kerana pendekatan rawatan untuk ketagihan candu telah berkembang dari semasa ke semasa. Walau bagaimanapun, idea untuk menyediakan penjagaan dan sokongan khusus untuk individu yang bergelut dengan ketagihan kekal sebagai aspek penting dalam menangani isu kesihatan awam ini.
5. Sejak tahun 1927, Pentadbiran Keretapi Tanah Melayu di Port Swettenham menggunakan trak berusia 10 tahun sebagai ambulans untuk mengangkut orang yang cedera ke Hospital Daerah di Klang. Trak ini juga digunakan untuk tujuan lain, seperti menghantar juruwang dan pengiringnya untuk tujuan perbankan. Orang ramai mempersoalkan sama ada Pentadbiran Kereta Api mampu menggunakan ambulans moden dan bukannya trak lama memandangkan ini adalah pelabuhan yang sibuk dan tidak ada ambulans lain yang tersedia semasa tempoh perbankan jika kemalangan berlaku.
Penutup
Pembinaan Hospital Daerah Klang yang kekal membolehkan kerjaan menumpukan tanggungjawabnya untuk menyediakan perkhidmatan perubatan di samping menaiktarafkan status kesihatan masyarakat. Rakyat bawahan terutama penduduk kampung, estet dan lombong mula meyakini perubatan moden di hospital-hospital kerajaan.
Kadar kematian dalam kalangan pesakit mula menurun, sekaligus meningkatkan jangka hayat penduduk. Beberapa jenis penyakit seperti Beri-beri, kusta, kolera sudah dapat ditangani dengan wujudnya berbagai-bagai perkhidmatan dan wad khusus di Klang dan kawasan sekitarnya.
Kehadiran imigran yang ramai dapat ditapis taraf kesihatan mereka sejak di atas kapal lagi agar masyarakat umum di Klang terhindar daripada potensi wabak yang merebak. Wabak malaria dapat ditangani dengan ilmu pengetahuan tentang sanitasi serta kebersihan persekitaran.
Sebelum Perang Dunia Kedua lagi, tiada kebanjiran para imigran dilaporkan di kem kuarantin di Pelabuhan Swettenham menyebabkan kem tersebut ditutup kecuali untuk kemasukan binatang ternakan yang diimport dari luar negara. Wallahualam
